10/26/2023 EnchantaVaughn Approved Behavior: QMHPA greeted Kaila upon arrival to the home setting. Upon arrival to the home setting, Kaila appeared to be in a content mood evident by her facial expressions such as her engaging in general conversation with QMHPA upon arrival. QMHPA informed Kaila that this session was going to focus on her ISP goals. Intervention QMHPA began the session inquiring about Kaila’s current thoughts, feelings and emotions. QMHPA discussed with Kaila eliminating harmful acts towards others and displaying sympathy towards family members. QMHPA implemented Kaila use journaling, breathing exercises or reading to help reduce anxiety. QMHPA discussed with Kaila the importance of being able to express her feelings in an appropriate manner and learn to accept positive criticism. QMHPA conducted a skills building activity. QMHPA explained to Kaila that the purpose of this activity was to assist her with listening to positive criticism without lashing out. QMHPA demonstrated with Kaila effective relationship techniques through role play by demonstrating appropriate communication skills. QMHPA explained to Kaila about learning to listen then respond with positive feedback and controlling hostile behaviors. Response: Kaila appeared to be receptive to QMHPAs’ interventions and services that were provided to her during the session. Kaila said she was frustrated when it comes to certain things about her that she doesn’t know how to handle. Kaila verbalized to QMHPA that her biggest problem and depression comes from her son and family members. Kaila verbalized to QMHPA that she doesn’t think that she is going to ever get back to herself if her children are still in her life. Kaila said she will not allow no one to talk to her any kind of way and she is hard to shut down because she want the last word. Kaila was able to communicate in a proper manner and displayed appropriate listening skills. Plan: QMHPA will educate Kaila on utilization of relaxation skills that consist of muscle relaxation, guided imagery, and slow diaphragmatic breathing. QMHPA will assist Kaila with coping and calming strategies to reduce depressive symptoms during individual sessions (1-2x weekly).
